Seeing the Home: AI to Find the Fall Risks Assessors Miss

Abstract: Most decisions about whether an older adult can stay safely at home rest on an in-home assessment. These assessments vary by assessor, checklist, and state. In our Los Angeles pilot of 300 homes, 32% of coordinator ratings placed members in a lower fall-risk band than the HOME FAST reference standard. Stairwells were the most common miss, often at the front entry. Cara is an omnimodal AI platform that runs on any phone with no app. It uses photos, voice, and SMS to capture home hazards, screen for depression and anxiety, review medications, and score ADLs, then turns this into structured data for care teams and health plans. This talk covers what we learned from scoring real homes, where human assessment breaks down, and how industry and university partners can build the validation evidence this field needs.
